Can you find the errors in this paragraph?

Objective: His weight today is 183 which is down 4 and 3/4-pounds since yesterday. Blood pressure today is 148/60 on recheck 144/64, pulse is 64 and regular. Overall this gentleman does seem to be better, however, his skin is still somewhat pale and almost ashen. He does have some mild JVD. He does have a slightly positive hepatojugular reflux. The swelling in his hands seems to have markedly decreased from yesterday. Heart Rate: S1 S2, somewhat irregular. He does have a grade 1-2/6 murmurs so on the lower left sternal boarder in the apex. The lungs he continues to have a few fine basilar rales in the posterior lower lobe.
